The Columbus Organization to Provide Case Management Services in Vermont

BLUE BELL, PA — The Columbus Organization has been chosen by Vermont’s Department of Aging and Independent Living (DAIL) to deliver home and community-based case management services for the state’s Developmental Disability Services (DDS) and Brain Injury Program (BIP). This partnership, part of Vermont’s HCBS-COI Project, will begin on October 1, 2025, with the potential to support up to 4,500 individuals statewide.

Known for its innovative, person-centered care coordination, The Columbus Organization brings a comprehensive approach to case management. The company combines advanced analytical tools with a strong commitment to personalized planning to enhance health outcomes for individuals and families.
