Columbus Organization to Provide Case Management Services for Georgia’s Seniors and Disabled Adults

BLUE BELL, PA — The Columbus Organization has been chosen by the Sowega Council on Aging (SCOA) Area Agency on Aging (AAA) to deliver home and community-based case management services, signaling a significant step toward enhancing the lives of seniors and disabled adults in Georgia. This collaboration will further broaden Columbus’ presence in the state, empowering older individuals to lead healthier, more independent, and self-sustaining lives.

The need for such services is significant, with over 60,000 seniors and disabled adults in the region under SCOA’s coordinated care. Columbus’ involvement will help address this unmet requirement, providing crucial case management services for these individuals.

Since 2003, Columbus has been working in partnership with the Georgia Department of Behavioral Health and Developmental Disabilities (DBHDD), currently serving over 4,300 individuals in the state. Their long-standing and successful history in the state has enabled them to integrate innovative, person-centered, data-driven approaches to decision making. Their operational model focuses on risk reduction, resource optimization, cost savings, and process improvement, ensuring they can effectively meet the needs of individuals.

Speaking about the announcement, Carlos Hernandez, Senior Vice President Care Coordination for Columbus, said, “For 20 years, we have sought to bring the most innovative approaches to support coordination in Georgia, and we have established a proven track record for improving the lives of individuals with traditional, intensive, and complex needs. We are thrilled to offer our services to the large elderly and disabled adult population and believe we will bring a unique approach to case management that improves outcomes for this population in need.”

The Columbus Organization currently offers case management services in multiple states and will begin offering services under this Home and Community-Based Services (HCBS) Case Management contract in Georgia starting July 1, 2024. This new partnership promises to enhance the quality of life for thousands of seniors and disabled adults in Georgia, setting a new benchmark for person-centered care in the state.
