PALTmed and LTC ACO Launch Value-Based Care Joint Alliance

LTC ACO and Third Eye Health Offer Virtual Care Network to Long-Term Care Patients, Facilities and Provider Groups

KENNETT SQUARE, PA — The Post-Acute and Long-Term Care Medical Association (PALTmed) and LTC ACO have announced the launch of the Value-Based Care (VBC) Joint Alliance, a collaborative effort designed to assist long-term care and assisted living providers in navigating Medicare’s value-based care programs. This initiative aims to improve care quality, optimize participation in Medicare Shared Savings Programs (MSSP), and enhance opportunities for shared savings.

The Centers for Medicare & Medicaid Services (CMS) plans to transition all Medicare fee-for-service beneficiaries into value-based care models by 2030, but long-term care providers often face hurdles such as financial, operational, and technological constraints. The VBC Joint Alliance seeks to address these challenges by offering tailored resources, tools, and guidance to equip providers for success in value-based care environments.

The Alliance includes access to a specialized online library featuring expert guidance on beneficiary attribution, clinical performance, cost optimization, and quality enhancements. Created with input from LTC ACO’s extensive experience, the educational resources aim to prepare providers to maximize the benefits of Medicare’s value-based care programs. Since 2018, LTC ACO has generated over $130 million in shared savings, demonstrating its effectiveness in guiding care providers.
