KENNETT SQUARE, PA — LCH Health and Community Services has been awarded a second-year grant from the AstraZeneca Foundation’s Creating Health Access for Next Generation Equity (CHANGE) program. This grant will provide essential funding to enhance healthcare access and promote equity for individuals facing barriers due to race, ethnicity, socioeconomic status, and other factors.
The funding will enable LCH to focus on several key initiatives aimed at improving community health. These efforts include increasing access to behavioral health services, providing care management for chronic conditions, and expanding partnerships with local schools to reach children and families. Additionally, LCH plans to bring healthcare directly to residents through community health screenings, ensuring care is accessible to those who need it most.
Acting as a vital safety net, LCH connects families with critical support services, such as health insurance enrollment and referrals to specialists, along with partnerships to address food and housing needs.
